The "In Family" programme regulations

 

§ 1.

General provisions

1.    The organiser of the "In Family" programme (hereinafter referred to as the "Programme") is "Fundacja Koprowski Dzieciom" located in Łódź, Skierniewicka 3/5, 93-114 Łódź (hereinafter referred to as the "Foundation").

2.    The Programme is valid from 13 April 2010.

3.    The Programme is organised and carried out within the boundaries of the Republic of Poland.

4.    The Programme is organised and carried out under the terms specified in these Regulations and according to the general legal regulations currently in force.

 

 

§ 2.

Aims of the Programme

1.   Aims of the Programme include:

•    creating family-type children's homes

•    supporting existing family-type children's homes

•    promoting the idea of family-type children's homes

 

§ 3.

Programme participation

1.    Families living within the territory of the Republic of Poland can be beneficiaries of the Programme.

2.    All requests for help and support must be addressed to the Foundation only in writing (letter).

3.    The Foundation considers requests for help and support only from those facilities which have supplied a complete set of the required documents, i.e.

­ completed application form

- certificate of the status of a family-type children's home

- income certificate

- signed consent declaration for processing personal data

- signed consent for making use of the image

4.    The Foundation considers requests for the construction of a family-type children's home from those persons who have delivered:

- a complete set of documents which qualify them for the role of foster parent

- abridged copy of marriage certificate

- income certificate or financial disclosure statement

- employer's opinion

- resume

- medical certificate of general health condition

- medical certificate from Mental Health Outpatient Clinic

- certificate from AA Centre

- certificate of confirmation that no criminal record exists

- confirmation of the completion of special training for foster family candidates organised by an authorised facility and the certificate confirming the award of such a qualification

- a signed declaration of consent to make their personal data available to the Foundation and have their image used for the needs of the Foundation

5.    In special cases, the Foundation reserves the right to request additional documents due to the given request's specificity or the manner of providing assistance by the Foundation in a given case.

6.    In special cases not specified above, the Foundation reserves the right to provide assistance on other terms agreed upon during a Management Board meeting.

7.    The Foundation reserves the right to refuse the provision of assistance even though the applicant’s letter or documentation indicates that all the conditions determined in the articles of association and these Regulations have been met.

           

§ 4

Programme Information

The Regulations, any information about the Programme not included in the Regulations, as well as the documents mentioned in the Regulations are available for inspection at the Foundation's head office and on the Foundation's website.

 

§ 5.

Amendments to the Regulations

The Foundation is entitled to change these Regulations during the term of the Programme.
